Student Insurance Information


Eligibility:

  • Domestic students
    • All domestic students taking 5 or more credit hours (3 hours in summer), all domestic graduate student taking credit hours, and students seeking a doctorate degree are eligible to enroll in this insurance plan. Eligible domestic students who do enroll may also insure their dependents per eligibility guidelines.
  • International Students
    • All registered international students taking credit hours and their dependents are required to purchase this insurance plan at the time of registration.

Please visit the website to buy insurance online or to view and print the entire plan benefits. Premiums will be billed to your SDePay account.  Open enrollment for Spring 2009 or Spring/Summer 2009 will go live October 31, 2008.  The deadline for enrollment period to be announced.

For Student Insurance Participants

If you have the student insurance plan, and you become ill, sick or injured. You should:

1) Report to the SDSU Student Health Services for treatment and/or referral. (health office hours) All covered medical expenses provided by SDSU SHCCS are covered at 100% with no deductible. No SHC referral required by Medical X-ray Center; Sioux Falls, SD. ASHC referral for outside care is not necessary only under the following conditions:

  • Medical Emergency. The student must return to SHCCS for necessary follow-up care.
  • When the Student Health Clinic is closed.
  • When service is rendered at another facility during break or vacation periods.
  • Medical care received when the student is more than 50 miles from campus.
  • Medical care obtained when a student is no longer able to use the SHCCS due to a change in student status.
  • Maternity.
  • Psychotherapy.

2) File claims within 90 days of injury or first treatment of sickness. Bills should be received by the company within 90 days of service.
